Waiver Form

All persons entering Eagle Skate Park must have a signed waiver form. NO exceptions to this rule will be made. Children under the age of 18 years old must have a parent/legal guardian sign their waiver. Children 6 & 7 years of age must be accompanied by a parent/legal guardian at all times.

You may obtain a blank waiver form at the front desk of the Wm. “Bill” Austen Youth Center, or by clicking on the link below. Please note that legal size paper (8 ½”x14”) is required in order to print the complete form.
